What causes this condition? — Visual acuity is the main function of the eye, our visual analyzer. Through the visual analyzer, the brain receives most of the information about the world around us. Visual acuity characterizes the resolving ability of the eye to see separately two points as closely located as possible. This ability of the eye depends on its optical power, and that, in turn, on its anatomical structure: on the curvature of the surfaces of the natural optical media of the eye - the cornea and lens, as well as on the length of the eyeball, the size and density of the cones - the receptors that perceive light , – in the area of ​​the macula. If these factors correspond to each other, that is, they are proportionate, then visual acuity will be as high as possible - 1.0 or higher. However, due to the fact that the optical power and anatomical features of the two eyes are not identical, the visual acuity of each of them may not be the same. This condition is called anisometropia. In addition to the factors already listed, visual acuity is influenced by the diameter of the pupil, the elasticity of the lens, the condition of the retina, its vessels, the optic nerve, and the person’s age. Depending on the state of all of the above, visual acuity can change not only throughout life, but even during the day. - In this case, at what time of day is it better to check visual acuity? Does this matter for obtaining a reliable result? — I wouldn’t talk about any strict time frame. How you feel at the time of diagnosis is important. If a person did not sleep well the night before, then in the morning it may be difficult for him to withstand the procedure of a complete vision examination. There are people who feel better in the afternoon, and there are those who, on the contrary, are invigorated in the morning, and have zero activity in the evening. But, perhaps, I would definitely recommend to everyone not to carry out vision diagnostics at least after 20 hours. Especially after a hard, stressful day, after long visual work at medium and close distances. I also do not recommend selecting glasses if you feel unwell, your blood pressure has increased or, conversely, decreased, your blood glucose level has increased due to diabetes, after acute stress, regardless of the presence of general diseases - after drinking alcohol. In addition, it is not advisable to check your vision if there is an inflammatory process in the eyes, a cold with signs of intoxication, with elevated temperature or residual effects after it. In all these conditions, the body is under stress and mobilizes all its strength to overcome it. And it can be difficult to concentrate when the optometrist conducts tests that require the patient’s participation and an adequate assessment of what is happening. All of the above can, to one degree or another, distort the research results, and based on this, a specialist can draw incorrect conclusions and write an incorrect prescription.


— I heard that the consequences of diabetes mellitus can sometimes provoke changes in the retina. - This is true. Changes in the retina caused by diabetes mellitus can periodically significantly change the refraction of the eye. In my practice, in such patients, the refraction with an increase in glucose levels and with its normal level differed by one or two and even three diopters. The insidiousness of this disease is that it is asymptomatic for a long time, and a person may not even suspect it. And the level of glucose in the blood can increase in response to the slightest stress, when the diet is violated, and even simply after eating any food or lack of sleep. This is why it is important to tell your optometrist about common medical conditions or symptoms that may indicate diabetes. — What else, besides general health and existing diseases, can affect the results of a vision test? — Often, people, while waiting their turn at the optometrist’s office or on the way in transport, actively use a smartphone or read a regular book or magazine. So, doing this is extremely undesirable, since you provoke excessive stress on accommodation and convergence. This condition is very difficult, and sometimes impossible, to remove immediately during the diagnostic process; as a result, the risk of error increases. Therefore, if you came for diagnostics in the evening, feeling unwell, and even if you read before entering the office, then most likely the optometrist will ask you to come again to assess the state of your vision over time. In the salons of the Optic City network, for example, in this case, a repeat visit is free for the client. — How to check visual acuity? — If we talk about testing only visual acuity, then various control tables are used to determine it. It is better if the signs of such tables are projected onto the screen via a projector or displayed on a special electronic screen. The average visual acuity is 1.0. For many people, this norm is higher than 1.0, so a specialist should always check the maximum possible visual acuity for a particular person. The proper functioning of the entire visual system depends on this, which, in turn, is the prevention of certain eye diseases. — I wonder what measures should be included in the procedure for selecting means of optical correction of altered visual acuity? — The selection of means of optical correction of altered visual acuity includes the following activities: external examination of the optical media of the eye, mobility of the eyeballs; checking the visual acuity of each eye in its natural, habitual state; determination of the leading eye and identification of obvious and/or hidden strabismus; determination of the optical power of each eye by an objective method - for example, using autorefractometry - and a subjective method, which include, in particular, the method of clarifying the correction using a trial frame, trial lenses, Jackson cross-cylinders or a phoropter, which combines all of the above, a control tables with special signs and a number of manipulations and tests; equalizing the balance between the eyes so that the glasses are comfortable and the visual system works in harmony; determination of the state of accommodation, which is the functional ability of our eye to see at different distances due to certain mechanisms, and in the work of accommodation an important role is played by the visual muscle, which in the specialized literature is called ciliary; determining the need for a “plus” supplement for visual work at close distances; assessment of the ability of both eyes to converge, that is, to bring the eyes to the nose when looking at close distances; trial wearing of glasses to assess the quality of vision, comfort and select the optimal correction. And this is just the minimum amount. It is often necessary to conduct extended research in atypical situations. In my practice, every second person has to expand its volume. In any case, in order for the selected correction means to be clearly visible and comfortable, so that adaptation is quick and easy, it is very important for us, specialists in the selection of optical vision correction means, to know the history of the development of decreased visual acuity, as well as whether other types of optical correction, and if so, which ones, for how long, are there any chronic eye diseases and general diseases, what medications, how often, in what doses does a person take and much, much more. This is why we ask a lot of clarifying questions during the vision examination process. — You said that autorefractometry is an objective method for determining the optical power of the eye. Can measurement data from autorefractometers replace a prescription? — An autorefractometer is a device for assessing the maximum, so-called dynamic or manifest optical power of the media of the eye - the cornea and lens - under conditions of current accommodation. The measurement results of this device are influenced by several factors. Firstly, the device itself provokes the occurrence of so-called instrumental myopia, due to the fact that, despite the imitation of looking into the distance, the subject still looks as if into the camera, at a short distance. Secondly, the state of the internal - ciliary - muscle of the eye is important, which provides accommodation and is therefore in a tense state, often excessive and asymmetrical, creating false myopia of varying degrees. The third factor is the position of the subject’s head; for example, uneven, asymmetrical and not at the proper level of fixation on the device support leads to the occurrence of false astigmatism. Finally, pupil diameter: a pupil that is too narrow or wide can change the true optical power of the eye. In this regard, the readings from the device cannot be interpreted as a prescription for glasses. It is necessary to use certain methods and tests to exclude instrumental myopia, false astigmatism, and excessive tension of the ciliary muscle. An autorefractometer helps reduce time and provides approximate data from which we base ourselves when examining vision. A competent optometrist can do without it. — You mentioned above that special tables are used to test visual acuity. For adults - these are tables with letters, for preschoolers - with images of objects and animals. The tables have 12 rows of characters of a certain size; from the top row to the bottom they gradually decrease. If the patient sees all rows, then visual acuity should in theory be equal to 1.0. Is it so? By how many units of visual acuity do the rows differ? That is, if I see, for example, only the fourth line from the top, what is my visual acuity? Internet users are often concerned about this issue. — Before answering your questions, I want to explain the principles of constructing tables for vision testing. There are different systems for measuring visual acuity: arithmetic, logarithmic, logMAR. How are they different from each other? The arithmetic system uses the decimal system according to the principle of arithmetic progression, that is, the visual acuity scale changes in the same step of 0.1. In Europe, the logarithmic system is considered the gold standard. It is based on the principle of geometric progression, that is, each successive step of change in visual acuity is calculated using a coefficient of 1.25. Measuring visual acuity on a logarithmic scale is more convenient. Firstly, in the case where visual acuity is below 0.1, it allows it to be measured in hundredths, which is important when studying visually impaired patients. Secondly, it is observed that there is no significant difference within the two visual acuity ranges, namely 0.6–0.7 and 0.8–0.9. Therefore, you can save time by not stopping at checking visual acuity in the lines corresponding to visual acuity 0.6 and 0.8. The logMAR scale uses the logarithmic value of the minimum angle of resolution. Using this system, it is possible to calculate accurate visual acuity even in cases where the patient sees only individual letters in a line. In Russia, tables based on the arithmetic and decimal system are mainly used. If, according to such a table, a person sees 10 lines from a certain distance, let’s take 5 meters, then his visual acuity is 1.0, if there are 12 rows, then 1.2. Accordingly, the third and fourth visible lines indicate visual acuity of 0.3 and 0.4. — Why should visual acuity testing begin with the right eye? — Most often, most specialists do this so as not to confuse the data from the study of both eyes. Although there are specialists who begin to study vision from the leading, or better seeing, eye. This requires increased attention so that when writing a prescription you do not make a mistake or confuse the indications for the right and left eyes. Therefore, it is easier to bring the procedure to automation in order to eliminate errors. In any case, all ophthalmologists and optometrists, when recording the results of the study, in formulating the diagnosis, and when indicating the optical correction in the prescription, first describe the right eye. An exception may be in the formulation of the diagnosis: the diagnosis that is most significant and dangerous for the organ of vision may be indicated first. — On what basis can we conclude that the patient has problems with binocular vision? — Binocular vision provides fusion, that is, the ability to merge the visual images of each eye into a single visual perception. Thanks to this ability, we see a single three-dimensional image of an object, we can estimate its size, the distance to it and between it and other objects. This is a very complex process; it is regulated by the visual center, which is located in the occipital part of the cerebral cortex. Most often, impairment of binocular vision occurs with reduced vision; it can lead to the development of strabismus - hidden or obvious. Impaired binocularity can subsequently cause a lot of troubles, such as rapid visual fatigue, inability to perform visual tasks, restrictions in choosing a profession, activity, or hobby. If obvious strabismus develops, most people lose the ability to see the world in three dimensions. To identify this disorder, a number of diagnostic studies need to be carried out. Often a person does not feel a violation of binocular vision until a certain moment. Such phenomena as uncorrected anisometropia, which was discussed above, can indirectly indicate a violation of this visual function; headaches, especially worse after visual work; frequent redness of the eyes, feeling of dryness; decreased visual performance, rapid fatigue - not only visual, but also general; desire to close one eye. If you notice that it has become difficult to read at close distances, and you get tired quickly, then perhaps there is not only age-related farsightedness, but also/or a dysfunction of binocular vision. Therefore, regular examinations by an experienced ophthalmologist or optometrist are important. — What are the methods for diagnosing astigmatism? Can a person suspect it himself? What are the symptoms of astigmatism? — Astigmatism is a type of vision defect associated with uneven curvature of the cornea and lens, as a result of which the image appears distorted and unclear. This condition must be corrected using optical correction devices. People with astigmatism may notice that they see better either vertical lines, horizontal lines, or diagonal lines. This can be verified using a radiant figure*. ____________ * The radiant figure is a round white board in the form of a dial with a diameter of 18–25 cm, on which thick (about 0.5 cm thick) black rays are applied every 10–30°. The ends of the beams are indicated by numbers (either on an hour scale or on an inverted TABO scale). The radiant figure is shown to the subject from a distance of 5–6 m in the form of a table, a translucent display or using a projector. Uncorrected astigmatism, in addition to reducing visual acuity, can provoke disruption of the accommodative apparatus and extraocular muscles, and the function of binocular vision. These disorders can manifest themselves in different ways - in the form of headaches, pain in the eyes, redness of the mucous membranes of the eyeball and eyelids, increased visual fatigue. All these are phenomena observed in cases of impaired binocular vision. Therefore, I repeat, it is very important to regularly check the condition of your vision and visual functions. And if an optical correction specialist recommends wearing glasses and/or contact lenses, then there is no need to treat correction tools as a “crutch.” Properly selected optical correction creates conditions for the physiological and/or coordinated functioning of the entire visual system, without stress, which will extend visual longevity. The second exercise can be performed after the moment of “flash” (clear vision of the letters) is two to three seconds, and to read the inscription you no longer need to blink often.

How does a person with such vision see?

Indicators of minus vision with a numerical value from 1 to 3 refer to low degree myopia. In general, myopia is characterized by poor distance vision - objects look unclear, their contours are blurred, a person cannot detail the picture and see small nuances. A mild degree of myopia is characterized by slight blurriness, while near vision remains good.

A person with such myopia without glasses may well:

  • read and write;
  • work on computer;
  • perform minor manual work;
  • navigate in space.

The person does not experience any particular difficulties in everyday life. But if a visual task arises “at a distance,” you have to wear glasses. With myopia, without optical aids, it is difficult for the patient to distinguish the numbers of approaching public transport, distant price tags in a store, look at the TV screen and recognize the faces of passers-by.

Some patients diagnosed with “vision -3” are interested in what percentage this is. It is immediately necessary to clarify that ophthalmologists do not measure sharpness as a percentage, but focus exclusively on diopters. But among the common people it was the concept of percentage that took root. It can only be determined conditionally if we take one row of Sivtsev’s table as 10%. At minus 3 diopters, a person sees on average 3 lines, so such vision can be designated as 30 percent.

Symptoms

Often, to diagnose a pathological condition, a patient needs to understand how people with vision minus 2 see. The following symptoms clearly indicate that there is a disease:

  • It is difficult to see the features and details of an object located at a certain distance.
  • The contours of things, houses, people, furniture, and other surroundings become blurred.

If this is not possible, then it is necessary to maintain the norms for the distance from the monitor to the eyes, depending on the size of the monitor, as well as the temporary norms for watching TV and working at the computer. Treatment

The treatment for myopia in children depends on the severity of the disease, the speed of its development and the presence of complications. The main goal of therapy is to stop or slow down the development of myopia, prevent complications and correct vision

Particular attention should be paid to progressive myopia. The child will have a better chance of maintaining his vision if timely measures are taken

The permissible limit for visual impairment per year should not exceed 0.5 diopters.

Treatment of myopia in school-age children is carried out comprehensively. The ideal option is a combination of physiotherapeutic treatment, gymnastics for myopia in children and medication. At the most serious stage of myopia or rapid progression of the disease, surgical intervention is also required.

Initially, the doctor selects glasses for the child. This is not a treatment, but only helps to correct vision. If a child has congenital myopia, glasses should be chosen in preschool age. For mild or moderate myopia in school-age children, the ophthalmologist prescribes glasses to help them see into the distance. You don't need to wear them all the time. Glasses should not be removed if the child has high or progressive myopia. For older children, contact lenses can be used.

For mild myopia, the ophthalmologist may recommend wearing “relaxing” glasses - they contain lenses with a slight plus. Thanks to this, accommodation relaxation can be achieved.

Training the ciliary muscle provides a good effect. To do this, lenses with positive and negative values ​​are substituted in turn.

There is a method of hardware treatment for myopia in children.

It means the following:

  • Vibration and vacuum massage. With its help, the eye muscles warm up before starting treatment with the device.
  • Color pulse therapy. Thanks to her, emotional stress is relieved.
  • Macular stimulation - stimulates those areas of the brain that are responsible for vision.
  • Electrical, laser and video-computer stimulation. Due to them, visual capabilities increase, myopia stabilizes, and tension is relieved from the eye muscles.
  • Electrophoresis. Drug electrophoresis using Dibazol or a myopic mixture has a positive effect on the eyes. It contains calcium chloride, novocaine and diphenhydramine.

If parents are interested in the question of whether it is possible to cure myopia in a child using hardware treatment, then we must admit that this is impossible. Hardware treatment does not affect the degree of myopia and is effective only with a slight deterioration in vision - up to 2 diopters.

An ophthalmologist prescribes eye vitamins for myopia in children at any stage of the disease. If the degree is weak, you need to take complexes containing lutein. In order to stop the development of the disease and avoid complications, calcium supplements, nicotinic acid, and trental are prescribed.

If the disease progresses too quickly or complications appear, the ophthalmologist prescribes scleroplasty. The basis for it is myopia of 4 diopters, which can be corrected, sharply progressive myopia, and a rapid increase in the anteroposterior size of the eye. Is it possible to understand how much “minus two” is a percentage? The number of percentages depends on how many lines out of ten the eye sees on a special table by which vision is determined. Each line represents 10% of 100% vision. There is no correlation between the number of diopters and "percentage" . Ophthalmologists have a set of lenses that can be used to correct vision. These lenses are selected individually. Two people with the same visual acuity may require lenses with different diopters.

How does a person with such myopia see?

A person will be able to read, work on a computer, or do manual work without much difficulty. However, with such myopia, the patient will constantly experience eye strain, fatigue will increase, and headaches may appear.

For a child, such problems can negatively affect his studies . Not being able to see the board clearly in the classroom will be a real problem for him. And increased fatigue will have a bad effect on a child’s learning, making him less effective. Therefore, children with myopia are recommended to sit closer to the board, at the first desk. You can also use vision correcting devices such as glasses.

In general, even such a relatively small degree of myopia can cause discomfort to a person. However, performing operations that do not require viewing distant objects will not cause any difficulties.

Causes
